Outreach Services

Empower Community Care Services also offer outreach services to adults with learning disabilities and the elderly. Our person centered approach empowers our service users in their quest for independence and social inclusion. Through cooperation with each service user a plan is formulated to ensure that there is progress in the journey towards the goal of living independently. Our staff team befriend, enable and advise. Help with self care, support with education and the ability to access community resources are also on offer.  Our aim is to empower our service users to develop their own skillset/strategies to flourish within their own home and community.
